Join author and presenter, Patricia O’Connor who will share her knowledge and vast experience supporting people and caregivers approaching end-of-life situations to unravel mysteries often encountered navigating that journey. She will share information in understandable language intermingling patient stories to shed light on what is often a medical maze.

Patricia O’Connor, will present wisdom from her new book Navigating Life’s Final Journey; Conversations, Choices and Resources.  She hopes to empower people by explaining the choices they have at end of life.

Patricia O’Connor, APRN MSN ACHPN has spent a career spanning 4 decades working in hospice, primary care and palliative care. Her work has always included caring for people with serious illness as they are approaching end-of-life. Certified in hospice/palliative care, O’Connor relocated to New Hampshire in 2015 to work at the newly forming specialty palliative care team at the Concord Hospital. She has spoken throughout New England and is currently a part of the Health Care Decisions Coalition – a group which is part of the NH Foundation for Healthy Communities whose aim is to advance the knowledge and use of Advance Directives and Physician Orders for Life Sustaining Treatment in the state of New Hampshire.
